2. Photocopy or certified true copy of the 1st quarter grade for the school term 2022-2023 for the ongoing Grade 12 students or last semester attended for senior high school graduates.

3. For Transferees, an Informative copy of TOR

4. Photocopy of Certificate of Good Moral Character

5. 1 pc 2X2 recent colored formal picture with a nametag

1. Wear decent and proper attire upon school entry.

2. No wearing of sleeveless, crop tops, shorts, tattered jeans, earrings for males, and slippers.

3. Wearing of face mask inside the campus is encouraged.
