3. PSA/NSO birth certificate/marriage contract-clear copy.
4. Transcript of Records for evaluation.
5. ID picture 2×2.
BISU Clarin Campus Graduates:
1. Photocopy of Transcript of Records/Diploma.
For Old Students:
1. Clearance from the previous semester.
