Implementation of Online Medical Registration for A.Y. 2021-2022, First Semester

Based on Memorandum No. 329 Series of 2021 issued by the University President III, all medical requirements have been waived for the meantime. In keeping with the new normal and taking into consideration the University’s policy on health and safety, you are required to accomplish the procedure enumerated below in lieu of your medical requirements:

1. Download the Student Medical Waiver found in this link: https://bit.ly/med-waiver-cla

2. Fill-up the Student Medical Waiver and affix your signature as well as that of your parent/guardian.(Make sure you understand the contents of the waiver, and tick only the medical requirements of your respective course/program. NOTARY IS NOT REQUIRED.)

3. Scan or take a picture of the filled-up waiver.

4. Fill-up the Medical History Form found in this link: https://bit.ly/med-form-cla

5. Upload the scanned copy or picture of the signed Student Medical Waiver at the end of the Medical History Form.

6. Click Submit.

7. A message will be sent by personnel from the Medical-Dental Clinic via e-mail as confirmation.
