Documentary Requirements

1. Must have complete grades from previous semester.

2. Approved Shiftee Form.

3. Approved Course Accreditation Form

For Item Nos. 2 & 3, secure the forms at the Registrar’s Office for approval.

Proceed to EDP for updating/posting of courses prior to online enrollment.
