Documentary Requirements

1. Certificate of Transfer Credentials/Honorable Dismissal (original copy)

2. Transcript of Records (Informative Copy/for Evaluation) (original copy)

3. Certificate of Good Moral Character (original copy)

4. Birth Certificate from PSA /NSO (clear photocopy)

5. 1 pc. 2×2 ID picture (formal)

6. Approved Course Accreditation Form.

For item No. 6, secure a form at the Registrar’s Office for approval.

Proceed to EDP for updating/ posting of course/s, prior to online enrollment.
